Effective on and after October 1, 2018, the Health Plan eliminated NVA as the vision insurance carrier. Alternatively, the Health Plan implemented a direct reimbursement to the participant for the eligible vision expenses.
In order to receive the reimbursement, simply complete the Vision Reimbursement Form and submit it, along with itemized receipts, to Wilson-McShane Corporation (a copy of the Vision Reimbursement Claim Form is included for your convenience).
As a reminder, the schedule of vision benefits (up to $200 per calendar year) was not changed. The benefits remain the same.
The only difference now is the process by which you receive reimbursement:

Heat and Frost Insulators and Allied Workers Local 34 was organized in 1913. For 100 years our Union has endured through good times and bad in peacetime and at war through economic depression and prosperity, and through it all, we not only have survived, we have flourished.
We proudly recognize all those who have played an important roll in our growth, and look to our ranks to continue this great tradition of organized labor, to work together to better the lives of all of our members to ensure that future generations of members provide the dedication of excellence that has brought our Trade to its position of excellence in the Building & Construction Trades Council of Minnesota.
